Our MyCare Program
Health doesn’t begin in a clinic, it begins at home. In neighborhoods, in routines, and in the social conditions that shape daily life. Our Community Health Worker (CHW) program addresses the drivers of health—social determinants, chronic condition management, and access barriers—by delivering trusted, relationship-based support directly in the community. We reduce risk, improve outcomes, and empower individuals to take control of their health where it matters most.
Our MyCare Services
Our trusted Community Health Workers serve as partners and advocates for participants, helping them take charge of their health and wellness. Services include:
Health Education & Coaching
One-on-one support to build the skills, confidence, and knowledge needed to manage chronic conditions and make informed health decisions.
Care Coordination & Navigation
Assistance accessing healthcare, social services, and essential community resources.
Community Connections & Support
Helping individuals build meaningful relationships and reduce isolation through community engagement.
Outreach & Personalized Support
Identifying unmet needs and connecting individuals with resources that can make a lasting difference.
Chronic Condition Support
Supporting individuals with chronic conditions to follow care plans and maintain long-term stability.

Who We Serve
MyCare is designed for children, adults, and families managing chronic health conditions or facing other high-risk factors. Participants may experience barriers related to housing, access to care, transportation, or other social determinants of health.
Services are delivered in the home and community to ensure comfort, continuity, and whole-person support.

MyCare services are currently available at no cost through a grant-funded partnership with Valleywise Health. Our team can also assist you with understanding Medicaid coverage options and future eligibility pathways as the program expands.
