Developmental Disability Home Care
Our LifeCare Program
LifeCare is built on a simple belief: independence is strengthened through the right support. We deliver personalized, home-based services that help individuals with developmental disabilities build skills, deepen community connections, and navigate daily life with confidence. Our role is not to direct the journey, but to walk alongside each person as they define and pursue their own goals.
Our LifeCare Services

Habilitation
Habilitation services are designed to help individuals develop and strengthen life skills that promote independence and confidence. We work one-on-one with each person on personalized, goal-oriented plans, supporting everything from communication and socialization to basic self-care and safety awareness.

Attendant Care
Attendant Care supports daily routines and long-term stability by assisting with non-medical tasks that help individuals thrive in the comfort of home. From meal preparation and light housekeeping to medication reminders and companionship, our team ensures a safe, supportive environment while promoting independence, consistency, and quality of life.

Respite Care
Caregiving is meaningful, but it can also be demanding. Our Respite Care services provide family caregivers with temporary relief while maintaining consistency and comfort for the individual receiving care.
Who We Serve
LifeCare is designed for individuals ages 3 and up with developmental disabilities, including but not limited to:
- Autism Spectrum Disorder (ASD)
- Cerebral Palsy
- Cognitive or Intellectual Disabilities
- Down Syndrome
- Epilepsy
All services are delivered in the comfort of home, avoiding the need for congregate settings and ensuring care is familiar, flexible, and person-centered.

Our Approach
At Home Assist Health, we believe in:
Self-Direction
Individuals are empowered to lead their care journey.
Independence
We provide support where needed, and autonomy wherever possible.
Connection
Community engagement is woven into our work, encouraging meaningful relationships and inclusion.
Purpose
Every service and interaction is rooted in helping individuals pursue what matters most to them.
Why Home Care Matters
Home care saves an average of $40,000 per person compared to hospital or facility care.
Parents are definitely the best people to care for their kids, so why not pay them to do it instead of having a third party come in? There are these resources out there to help take care of your child, and this is just one amazing resource.
